Community Emergency Response Team Application

Thank you for your interest in the Coral Springs/Parkland CERT team. This application is for enrollment in our team if you are a current CERT member moving to the area. It is required for you to provide all your credentials in order to become a member of our team. If you do not have your credentials you are welcome to retake the training at which time your application can be reviewed. You can conveniently and easily fill out this form and click Submit and your application will be submitted to our Leadership Team for review.  We look forward to having you on our team of volunteers. 

      
In order to join the Coral Springs/Parkland CERT team without retaking the training you must provide proof of all your certifications. You must upload all of the below required documents in order to be considered for enrollment. If you do not have all the below credentials then you must retake the training.

I, the undersigned, certify that there are no willful misrepresentations, omissions, or falsifications  in the above statements or answers. I understand that any omissions or false statements on this application shall be sufficient cause for rejection for enrollment or dismissal from the Coral Springs/Parkland Fire Department's CERT Team. I further give my consent to the Coral Springs/Parkland Fire Department to conduct a thorough background investigation that may include, but not limited to, any criminal history, employment history, and personal references. I also understand and agree that my participation on the team puts me in public view and my picture may be taken and posted on the internet or in the media.
